PHILADELPHIA CONFERENCE-2007 REGISTRATION FORM
A unique collaboration between Arizona Fathers & Families Coalition, Inc., and the Philadelphia Fatherhood Practitioner Network. AZFFC has been asked by Philadelphia Practitioners to bring our premier fatherhood & families’ conference to Philadelphia! We are excited about this national collaboration as AZFFC along with other nationally recognized fatherhood networks have established a new national network, ”National Network of Fathers & Family Services” headquartered in Phoenix and in Virginia, Boston, Bay Area, New York and DC. This conference is a true collaboration of the premier consultants on Responsible Fatherhood, Mentoring Children of the Incarcerated, Developing Promising Practices, Fathering Behind Bars and on the Street, culturally Competent Practices and Healthy Marriages.

Cancellations
Cancellations must be submitted in writing and received at least fifteen (15) days prior to the conference. A $100.00 enrollment charge will apply to all cancellation requests to cover hotel and catering costs. AZFFC will send an invoice for all cancellations after the deadline. No refunds will be made for cancellations received less than fifteen (15) days prior to the conference and by submitting this registration form all unpaid fees regardless of attendance will be invoiced at the post 21 September 2007 rate if unpaid after 8 October 2007 .
